Windows wont start

Hi,My friend recently did a disk cleanup and then defragged wand since then her PC will boot as far as the diagnostic screen and then a message comes on as follows: WIndows could not start because the following file is missing or corrupt.Windows\system32\conf\system.
you can attempt to repair this file by using the original setup CD.
she has tried this but only gets as far as the diagnostic screen again showing the same message
she tried accessing the bios but still got the same result.Also tried to boot in safe mode but still the same.
Is this a sign of HD failure or is there something she can do to format the HD and re install windows.

she has WindowsXP
