Try giving the mouse ball/contacts a good clean. The rollers in particular

WHAT COULD IT BE

Is it cordless? or perhaps Median?

Check for new Drivers, uninstall and try again

Take out the drivers and try it without. basic mouse functions don't require any specail drivers.

Some optical mouses/mice require calibration, see if there is an icon in the system tray or look in control panel.
