Formatting commands in emails

I have made up a Questionnaire in Word with checkboxes so that the respondent can click on it and check the box of their choice. I want to send this form via email but not as an attachment. When I copy the form into an email, the formatting is lost and the checkboxes disappear. How can I get round this?

Save it as HTML and make sure your email is also set to HTLM apposed to Plain Text

Of course, all your recipients would need to have their email clients set to receive HTML mail as well.

I've been messing around and the same thing happens for me when using Text boxes which I presume you mean. However, if you use two square brackets [] the whole thing will copy and paste. Your answers would look like this [x],but it was the only way I could get round it.

There will be some experts coming home from work soon that I'm sure will have a better answer.
