Connecting PC with other Network

I am having PC in my home and i want it to connect to the another network(for example: to my office PC). I want to access my home PC from my Office PC. There is any possible, if any tell me the way. Its urgent.

Thank you jack. But, its not working. I can't connect to my home PC from my office. But in the office network it works. Any solution?

With your Home PC running, online and with VNC running in Server mode, you should be able to access it over the internet by entering its IP address. The problem can be that, unless you remain online permanently or have a fixed IP address from your ISP, each time you connect you might be allocated a different IP address.

You also need to configure your firewall to allow access.
