It's often the case with tragic accidents involving safety critical equipment that there isn't a single cause, rather a number of errors going unchecked that lead up to the fateful consequences.

The Buffalo plane crash illustrates this principle.

I only see it as ONE or(two) "pilot error"

1) The pilot and co-pilot were poorly trained and inexperienced on the aircraft involved.

2) The co-pilot was not 100% fit and should have called in sick.

3) Idle chatter at that altitude was against the rules.

4) The plane shouldn't have been on autopilot in the conditions.

5) Failure to appreciate the severity of the ice build up and the correct application of the deicer mechanism.

6) Failure to respond to the stall warning correctly. The pilot pulled the stick back rather than forward.

(These errors may not all be highlighted in the single link I made.)

The correction of even a single one of which would probably have saved the flight.
