click here the Zalman coolers are generally accepted as being amongst the quietest due largely to that huge ornate heat sink, maximising passive heat dissapation.

Unfortunately the most silent and the most effective cannot be in the same product - it's all a matter of balance. I've used and like Titan thermostatically controlled fans - but I find these harder to get now, as my main wholesaler doesn't stock them.

For case fans, get ones with two ball bearings, and mount them on rubber washers to minimise noise. Once again I found Titan thermostsatically controlled ones better.
