Keyword & description meta tags - on every page?

Just a quick query, is it normal practise to include a keyword and description meta tag for every single web page on a website?
Or is it better to have them just on the index page?

I don't use meta tags any more - they aren't really necessary. Modern search engines use very sophisticated indexing techniques, and provided your site is well-designed, and doesn't have what the engines call 'inappropriate content' they'll index you automatically, without you having to do a thing. The web-bots will read your site content, so just make sure that your index page contains a good description of what you're about, and leave the rest to the engines.

It's important to pay attention to a couple of things by the way..........

1. Make sure the site navigation works perfectly

2. Ensure that every hyperlink is active - no dead links.

3. Update the site regularly and often - search engines hate sites which look dead in the water.
