They will only be able to open the document if it is saved in a certain way and if they have Word installed on their computer, and even then if you have Word 2000 or later and you don't save it as a Word 97 document, users of Word 97 will be unable to open it.
You can upload a Word document saved as an RTF (Rich Text Format) file that almost any word processing software can open and put some instructions to the web page about what you'd like your site visitors to do with it.
Create a folder in your web root folder and call it downloads. Use the File, Save As menu in Word to save a copy of your document in the downloads folder you just created but make sure you save the Word document as a Rich Text File in the Save as type option. Now create a link from your web page to the document and upload the page along with the downloads folder (and its RTF document) to your web host.
You may be better served by using a form to collect the information you are looking for.
