First stage should be to be to look at the data that is entered on the present form and ask yourself - do I need it? Is it relevent?. I find most forms that are over a couple of years old contain things that aren't really needed or are hangovers from previous procedures. Take out the dross and the things you are not supposed to ask on personel forms and then work on the flow of the form. The form should follow the task - no skipping backwards and forwards.
