Defragmenting confusion

Normally, I only defragment when the system tells me to. But today, two weeks after the last defragment, the analysis showed red stuff everywhere and told me to defragment.

Trouble is, I haven't actually done anything with my computer, nor been anywhere. How does it decide to need defragmenting, and how advisable is it to defragment fortnightly?

The only reason I decided to defragment tonight was because my every-day functions had become woefully slow - and, yes, I did virus/ad-ware-scans and couldn't find a sausage.
