Creating a form on a website

I'm looking to create a form on a website with some multiple choice questions and some name and address details which I want to be submitted to me via email.

Is this just a case of writing some HTML to do this, or do you have to have a database behind it?

Any good webpages with instrctions and maybe some code I can copy?

If you just want the forms to be sent to you via enmail, you do not need a database. You can, of course, add the detail to your own db or spreadsheet locally (i.e. input it manually to your PC).
Proprietary software such as FrontPage or Dreamweaver (and no doubt many other brands) make it very easy to create forms. However, you really need a utility to send you the form detail in a friendly, easy to read format, and such utilities usually require cgi-bin in which to run. I once created a form via FrontPage which was almost incomprehensible once it had arrived as an email. Keith
