Can anybody provide any "opt in" code

I have a website and am using some 3rd party software to create a squeeze page.

I've come to a section where I'm a bit stuck; I need some code to capture name and email address can anybody provide such so I can insert it in the application.

Leaving aside the issue of squeeze pages for now, is there any reason why you cannot simply use a small form with the requisite fields? Or is that not what you're after?

It's for an affiliate site which I've joined. I'm using some free 3rd party software which will generate a squeeze page which I can then use to get get my own customers.

Part of the 'freeness' of it is that there is a section to enter the code to capture name and email address which I will then plug into the application.

Not being a coder I've no idea what this would be.

In that case, you will need to learn how to create a properly formed HTML form, how to control its appearance with a bit of basic CSS, and then how to parse its data with PHP. I could give you code examples, but I don't want to post that quantity of code here in the forum, particularly that much PHP.
