Gaming PC - upgrade advice

Hi all,

I am hoping you can help me out. My gaming PC has not been updated for a while and I think it is in need of a pimping - most of it is 3 years old, and it cannot run Witcher 3 without disappointing lag. This might be down to an urgent need to reinstall Win 7 but is anyone able to advise if any of these components need to be upgraded? I am looking to spend ~ 500 USD / 300 GBP. What do you think is worth keeping and what needs to be replaced? Should I rebuild from scratch?

CPU: 6 core AMD Phenom II x6 1055T GPU : Asus NVIDIA GeForce GT 640 2GB (not crossfire apparently) MB: M5A99X EVO R2.0 (still OK I think - can handle crossfire) Mem: 8gb DDR3 at 667mhz (not noticed any bottlenecking due to mem) HD is not SSD

Windows experience - not the best judge I know - rates the HD as far less than the other components.
